It's not so much a hole as it is the fact that you are rubbing up against the curbs/invisible collision meshes in this particular spot within the prefab.
I've gotten similar tilting when rubbing against sidewalk curbs within the cities ingame.

Yep, this is strictly a ProMods feature.
SCS Software has never had this feature.

The images are assigned through the "looks" assigned to the model. Each look is a different image (or sets of images) that get called to load. That will require remodeling the Arlberg tunnel model assets.
The "cones" of light are part of the physical tunnel model.

It is my understanding that the "universal" folder means that the mod is version agnostic (will work on any version of the game).
Several of the Steam Workshop mods I use have version specific folders and those folders are only loaded by specific game versions by the "versions.sii" file settings.
