Hi, figured I'd add based on Bedavd's format.
NORTH CAROLINA
North Carolina has only rear plates.
This is the current, primary design for North Carolina. (note: there are alternate designs that are nearly as common but this is the primary design)

It follows an ABC-1234 format.
This is the North Carolina Apportioned design.

It follows an AB-1234 formatting (no specific changes between plates that I can notice)
This is the trailer plate (note: smaller trailers use a different design)

It follows an AB-12345 formatting.
EMERGENCY VEHICLES
North Carolina Highway Patrol uses the normal plates, except the formatting is SHP-1234 as opposed to the regular ABC-1234 (SHP standing for State Highway Patrol)
City police departments use the orange permanent plate, so do most other government vehicles.

Wisconsin uses both front & rear license plates

Alabama
Alabama ONLY uses front license plates

INDIANA
Indiana ONLY uses front license plates
This is the Indiana Current Standard license plate design:

It follows a 1233 ABC format.
As a Michigander, however, I don't think I've ever seen that plate! The VAST majority of Indiana plates I see are this design:

It follows a 1234567 format with APP in the top left and PWR in the top right. Interestingly, many Indiana Apportioned plates include the name and logo of the company that they are registered to.

Minor gripe, but the CA license plate needs to be updated, like NV's recently was. The new CA plate, (not so really all that new anymore) was introduced in 2011 and has the California DMV's web address on the bottom. The one in the game at the moment is the old 1994 to 2010 plate that lacks the website URL. For reference, here's a pic of of the current (new) one, with the web address emblazoned on the bottom:

While in game it doesn't matter that much, Colorado has had to change its format to Letter-Letter-Letter Dash Letter-#-# (ABC-D12). We finally ran out of combos from the pervious Letter-Letter-Letter Dash #-#-# (ABC-123). (Before that it was inverse.)
Colorado does indeed have a crap ton of plate choices. I personally have the state park plate in the old design. Most common plate outside of the standard though is the "Fleet Plate" which is red like the commercial ones. These get attached to rental cars and most other vehicles that belong to a fleet. Columbine is probably the 3rd.

Thanks for the note! I'll add that to the formatting in the CO post.
Another interesting thing about CO, and I believe California does this as well, is that as long as your plate is readable, you can keep it forever. My Uncle still has his old White over Green plate and he's told me random people have asked if they could buy it off of him lol. I'd love one day to get the non-standard designs but that isn't super important. My dream is still to see random out-of-state license plates...

Virginia is another state that allows drivers to keep their plates as long as they're still readable too. My car still has old plates that use a design that isn't issued anymore. I see lots of cars that have old plates, even on newer cars where the owner transferred the plates from their old car. The other day I saw a car with an old Virginia plate design that was phased out in 1993!
