Should miles be added to the distance signs?

I wouldn't put that much effort into it. That would be essentially putting in more effort than needed for a reault that tries too hard to be realistic, but ultimately fails.
Distance signs in reality rarely show the same number as a GPS. They generally show the distance to the center of town (but not always exactly the center), and the road it is alonged may change course over time. There are many variables that affect the total distance one drives before reaching this center point, so the signs are not exactly accurate.
I've also noticed that many of these signs are based on different points in the same town.
For example, if you are approaching a city on a rural road, the sign might show the distance to the main town square in downtown, while the sign on the freeway a few miles over might show the distance to a major interchange on the edge of downtown (a couple miles from town square). If you approached the city from the opposite direction, the distance signs on the rural roads might show the edge of town square closer to that side of town, or the same central point, while the freeway signs might show the exit of the interchange closest to that side of town. Town squares and interchanges can easily cover half a mile, or even a full mile, which is a large area to be the "center point" of town. The GPS might show a variation of several miles, especially when rural roads are involved going through several towns on the way.
One way around this is to set a GPS route from the sign to a point somewhere in the general downtown area, and use that number. Accurate to the city, but not so accurate as to be unrealistic, while also saving time and effort.

I disagree about the exit numbers. There's no need to change them as all the exists in the game represent an exit in the real world, and the numbers line up between the two. Even if the distance on the mileage signs doesn't match the exit numbers exactly, I'd be okay with that.

These sign templates were added in the recent patch. Maybe (hopefully) it means that they will be getting rid of the arrows and going to distance.

Hmmm. Perhaps this thread was beneficial after all. Looks great. I just hope its gps miles to (x) location. At least close to what our gps would say at least.

Well Mrazík did say that they were going to experiment and test different signs. That's probably what these are for. They might not have come to a final conclusion on what they're going to do yet.
