In the Spanish version, when you activate secondary names, this is what appears for these 4 cities:
Bilbao
O Barco de Valdeorras - O Barco
Lleida
Villarreal
We're entering political issues here, but if your game is localised in Spanish language and you've selected secondary names, this is what should appear:
SPANISH NAME - OFFICIAL NAME IN LOCAL LANGUAGE
Bilbao - Bilbo (Basque)
El Barco de Valdeorras - O Barco de Valdeorras (Galician)
Lérida - Lleida (Catalan)
Villarreal - Vila-Real (Valencian)
La Coruña - A Coruña (Galician)
Valencia - València (Valencian)
Maybe you should include PAMPLONA - IRUÑA as well, but I'm not sure about the officiality of this last one, you should check it out.

i checked the game data, and all local names are correct except vila-real (this also explains some mess in research we had, thanks).
bilbao is officilaly called bilbao even there (although basque name is different) so this might be something to discuss. much like pamplona/iruna.

Yes, what called my attention was O Barco de Valdeorras and O Barco, both names are written in Galician language, I noticed that if that city name was (intended to be) translated, why not the rest? But yeah, maybe this is something to think carefully before taking an action because it's a political issue.
