The mod manager showing a mod as green means nothing. You need to check the workshop description or the site where you got the mod from to determine if it's been updated (and even that's not always accurate).
Your game log shows files being converted from old versions, which means you have out of date mods. And if a modder is lazy enough to leave old files in the mod, they've probably made other mistakes.
Based on the location you've probably narrowed it down to mods that affect Poland.

@dialectus You are using the Hybrid RC so you can't use the ProMods-RusMap RC. Even if you weren't using the Hybrid RC you can't use both versions of the ProMods-RusMap RC together. Remove the two ProMods-RusMap RCs

Re: Index Outside Array Boundaries
There's still at least one out-dated mod in there:
00:02:12.233 : [prefab desc] Converting prefab descriptor '/prefab/roundabout/road2_med_4c_tex.ppd' version 22 to current version (23)
And then there's these.
00:05:04.394 : <ERROR> [traffic_vehicle_chain] Unable to find vehicle data 'traffic.transit_16.open_d_1'!
00:05:04.394 : <ERROR> [parked_vehicle] "Unable to parse allowed vehicle definition!" [41489.0;126.0;5020.2]
00:05:04.394 : <ERROR> [parked_vehicle] "No suitable vehicle model found!" [41489.0;126.0;5020.2]
I don't use map mods myself, so can't really help in narrowing down which ones it may be.
You could try starting a new profile and add one map mod at a time until you get the crashes again.
