Serious answer, there is a very old and rare bug in the code that makes AI traffic vehicles suddenly snap backwards (180°) while driving, or spawn backwards but with correct speed vector. If it happens to a semi, it can affect only the truck, or only the trailer, or both. Sometimes AI can just make a sick J-turn and continue driving like nothing happened. But more often, especially with larger vehicles like buses or trucks, it will hit something like other AI or guardrails or other infrastructure, and stop, blocking the road.
I suspect it happens because of some sort of value underflow or sign error somewhere in Euler angles or quaternion calculations, whatever the game uses for AI traffic models. It's not a common bug, and it's hard to catch it happening in real time, more often you just see the pileup of AI traffic in front of you with e.g. a truck with its cab inside its trailer, or two cars with one of them facing backwards, or a bus stuck like that screenshot on previous page. But it's been in both games for many years. And once in a while, you can see it. There's no obvious pattern to it, like certain roads or time or particular models of traffic. There might be a pattern, but it happens too infrequently to notice one.

I actually like those random accidents as they feels more "live" than the staged random events. I just wish the game could sense/register them and spawn a police car close

Yeah, I guess it's very rare, since I have over 1400 hours in ETS2 and it's the first time I saw anything like this.
